If I could select no stars I would. Since my last review a month ago they’ve taken away most of the medicines I need!! But they are vengeful and will take out a bad review on us patients. I’m pretty sure I’ll be dead before long!! They’re trying their best to kill me!! Doesn’t the state ever check on this place? Why are they allowed to operate? God help us all here!!
I thought Magnolia Manor was horrible but Brushy Creek has them beat!! So much incompetence from the top down. CNA’s are inept. I wonder if they don’t just hire people off the streets!! I lay all day on a soiled bed with no one checking on me! How are these people still open?! Everydyim trying to find ways to end my life so I can be out of my misery! Why don’t I move to another facility? They’re all the same!! God please take me!!
Very hard to get anything done on time. The nurses are understaffed they move from one building to the other took me 7 1/2 hours to get my medicine last night. I have just gone for it at 7:10 it’s 9 o’clock and haven’t seen a nurse yet wish I would’ve never come here. it is now 11:30 AM. I thought it couldn’t happen again but it did this time six hours and 45 minutes to get my medicine. Peachtree in Gaffney is 100 times better than this.

To our 3 Activities Ladies, Emory, Katelyn and M.J. Just to let you know, you all are so good to us and we love you all 3 dearly. You have become part of our families and do a great job of keeping us active and happy at Brushy Creek Post Acute. That means a lot to each of us, especially those of us that are permanent residents. Thank you so much for being so good to us. Love you all.

The patient ratio to workers is completely disappointing and causing lack of care for the elderly and sick

If I could give this facility a rating of zero, I would. It seems that either fake reviews were posted or there is racial bias in the treatment of patients. In February, my father was admitted for rehabilitation. At the time of admission, he had no open wounds, but after a fall in the bathroom, his condition declined. The facility is severely understaffed and overcrowded, with 12 patients per cottage and only one nurse and one CNA. When I discussed my concerns with the administrator, she provided false information, claiming that state regulations only permit one nurse, and one CNA per cottage. When in fact, the regulations require a minimum of one nurse, without prohibiting additional staff. A CNA was fired after sharing information with us, citing liability concerns. Although the staff is generally good, and provided the best care possible under the circumstances, the administration is a concern. We were informed that patients with wealth and status receive better care. The social worker for Rhododendron lacks empathy and only responds at her convenience, without taking initiative to help the patients and their families. As a former social worker, I knew first hand that going the extra mile means the world to families who are already stressed to the limit. The contracted rehabilitation services are inadequate, and patients show little improvement. After my father was readmitted to the hospital with an open wound sustained at Brushy Creek, the facility refused to readmit him, allegedly due to our inquiries and their potential liability. I believe this facility provides subpar care, particularly for people of color. They are simply pushing people in and out for financial gains. I actually would put a dog there.

Do NOT choose this facility for your loved one. In my opinion, the level of care here is completely unacceptable and raises serious concerns. During a five-day stay, my loved one received ZERO physical therapy!! The facility is clearly understaffed—only 2 nurses covering around 24 patients across 2 cottages. At one point, no staff checked on patients the entire time I was there. On another visit, there was only one CNA present. If there had been a fall or fire, no one would have been available. Basic care was lacking. There were no sheets on the bed, the TV didn’t work, the chair was torn with exposed foam, which is both unsanitary and unsafe, the toilet was leaking and never fixed. The room and bathroom were NEVER cleaned the entire five days—no housekeeping at all. They have 2 designated days for showers. Around 7:00 pm we asked if this was going to be done. The nurse said yes. At 11:00 pm my loved one was woken up to take a shower. Who does this to someone??? I also witnessed a patient sitting nude in their room with the door open for an extended period, with no staff addressing the situation. That level of neglect and lack of dignity is deeply concerning. I personally witnessed an EMS arrival where paramedics were walking around trying to find someone just to let them out of the building. That alone says everything about the level of organization and staffing. Another example—my husband arrived and no one let him in. I had to track down a nurse myself to get the door opened. Discharge was handled terribly. My elderly loved one was placed in the hallway with their belongings in a box, waiting all alone for family to arrive. There was NO assistance, NO dignity, and NO concern for a safe transition. There are no RNs on staff, only CNAs and LPNs, and while the nurses are kind, they are extremely overworked—some working from 3 PM to 7 AM. This is not safe for patients or staff. This place feels more like a holding cell than a care / rehabilitation facility—patients are essentially kept there and given three meals a day with little to no real rehabilitation or oversight. I truly feel sorry for patients who don’t have family members to advocate for them. For comparison, after transferring to another facility, my loved one had 4 therapy sessions scheduled the 1st day—proving just how inadequate the care was here. Please take the time to read other reviews as well—there is a pattern of similar negative experiences that should not be ignored. There appears to be a pattern of similar concerns in other reviews. I will be reporting this to the SC DEHEC and contacting the insurance provider regarding services billed but not provided. This facility is unacceptable!!! In my opinion this is insurance fraud.

My mother was sent here directly from the hospital to receive rehabilitation services to be able to get stronger in her walking and other vital tasks. She was never taken to the physical therapy room. They simply sat her on her bed and asked her to do aem lifts and had her walking around a little bit. The CNA‘s were very curt with her. The room was terribly small and ill equipped. She practically had to beg to get a chair in there for a visitor (and the chair was broken). when she asked if she could get a reclining chair to get out of the bed and be able to put her legs up, they simply told her to get back in bed and put her legs up that way. Their answer was that they did not have enough chairs to go around. The food was subpar and she was continually given the foods that she did not care for, even when she was asked about it. When she asked for something else, she was given a prepackaged honey bun. We will never consider this place in the future for any kind of rehabilitation services. Terrible.

All businesses, but especially ones of this nature are made or broken by its staff! Brushy Creek Jasmine has some amazing CNA’s and Nurses!!! Another resident’s daughter and I (sister of a long-term resident) were talking about Linda P. (Lynn) yesterday! Lynn does her job with her whole being! She is so hard working, always willing to problem-solve. But the most important thing we talked about was her creative way of connecting with ALL the residents to help them with their unique challenges! My sister has hallucinations that at times frighten her, and I have a hard time knowing how to help. Lynn creatively redirects her fears and worries and just makes everything better!!! I keep hoping that she will adopt me!!! Linda P. (nickname: Lynn) has been such an amazing blessing to my sister and our family. The other resident’s daughter shared a similar sentiment!!! I thank God for Brushy Creek and especially for people who do this type job with their whole hearts!!!

How can I put this. The very first time Brushy Creek it was so lovely it was the place to be. Omg. Was i wrong. And it all started over a wheelchair cushion.SHM!!! And some of the things she had made to another people. After all said and done I decided to speak to the social worker, and then she took it with her supervisor. We came to the occlusion that she stay away from me. So everybody started looking at me sideways including first shift nurses. They would pass out medical and be gone and you would not see them until it medicine time again are if a new admission come. Pt was awful but I did what I had to do. Social worker come to me and told me my time was up and I may have any over stay bill. Not what wrong this picture by this time red flags everywhere. Ok hear we go. If anything can get worse. Upon my release. Social worker was suppose to have got me a hospital bed. I tell you what ? You figure out. To all my CNA think you for being so nice and great to me. And big shout out all my CNA and that including Big shout my night shift Nurses. And a shout to the person that took my quilt. One out 10 I giving stuff a 1. Not you Dr. Allen i give you a 10 all the way. I think I worse out my welcome.

If you need to go to rehab to get stronger after an illness, this is the place to come. It’s so beautiful, it’s a new facility, they keep you active by providing things to do. The staff is so friendly especially, Kyle, Emory, Audra, Sara, my physical therapist. I am sure that I forgot some and I am sorry. It’s just that I have had so many people helping me.

My Grandmother was left with a bed sore that grew the size of your whole hand on the bottom of her back. The family was never notified. When we went to visit, an unbelievable smell came from down the hall. The closer we got to her room, the stronger it was. After walking into the room and almost vomiting, I realized it was my Grandmothers BODY. LITERALLY rotting from INFECTION. The most horrendous smell I have ever smelled. I asked my Grandmother what was happening and she could only tell me her “back was hurting”… I rolled her to the side a bit and saw a literal puss filled HOLE bigger than a softball! I have never seen a wound so horrific in my life. I immediately called my cousin who’s a RN to come straight there and she said the packaging looked to have not been changed for a minimum two days but overall she should have been sent to the Hospital long before it got in that kind of shape. She was also absolutely horrified, as we all were. The staff said, “We called the Doctor” and continued to blame the person prior to their shift for not doing something they said they did. To let a human rot so bad they have smelled up and entire floor of a nursing home. . She passed away within days of this. I don’t even have enough words to describe what I think of this place.

I am a friend or relative of a current/past resident My mother-in-law has dementia and has been a resident for at least 2-3 years, since her stay we can never get anyone to answer the phone so we can check on her. We live about five hours away from the center so we can’t visit as often as we would like to.
No one ever answers the phone so you can check on your loved one.

I was there 3 years ago for my heart and in November after major surgery. What I saw was complete incompetence. Seems the owner doesn’t pay enough to get professional nurses and can workers. They refuse to hire enough of them to take care of the number of patience they have. Read the reviews, it will scare you. It tells you they are working to make things better. How long does it take to hire competent people if you pay them enough to want to work there. Not the smart mouth people they have there now. Gave them a picture of nurse sleeping at 2something in the morning. Bet she is still there sleeping!! No meat for breakfast, just grits and powdered eggs. If you are diabetic, you all get the same meals. Usually carbohydrates and nothing a diabetic should eat. One time I got my heart meds at 12:30 at night because the nurse didn’t come in. I woke the cna? Up and she said she would call the nurse. When she did come she was dressed in fadedout sweet shirt and worse looking pants. This was 3 years ago. I won’t even get started about my stay in November past!!!There advertising didn’t match what I saw.why can’t they just be Honest!!

My mom was placed out here last week In Azalea Cottage, and a black African American man came into her room with out knocking ( he was wearing khakis and a blue shirt ) and then he went to say he was the night shift nurse. Not a Dr. But a nurse. Meanwhile the cna was in uniform. I thought it was so unprofessional ! While everyone has on a uniform. He was very rude to the CNA as she asked him for him !

NEVER SEND ANYONE HERE. We just endured eight weeks of pure hell with my mother-in-law. Before she arrived we got a call from Tabitha and was told she was our point of contact. After the first few issues we reached out to her only to be called be the nurse manager Jan who told us to never call Tabitha again bc she was only marketing and didn’t know anything. Asked for a therapy plan/schedule and it was NEVER received. She was non weight bearing and the whole point of therapy was to help strengthen her and when asked about her arm weakness we were told that wasn’t PT…..that was OT?!?! Rooms were dirty. Changed linens and swept and wiped down bathroom myself on several occasions. When asked for assistance to help transfer her we were told by the staff that they were off the clock that we would have to wait…..this happened with multiple staff members. The CNA are hateful and lazy. Several times we rang the buzzer to get in and had to stand outside 15+ minutes. Once we walked around to the back and the staff were sitting at the desk eating while we were waiting to get in. Forget any help from the social worker. Brynn would never follow up and could care less about the patients. Communication in The facility is a joke across the board. Nurses aren’t much better with the exception of Selena and Jessica. I once asked to speak with one regarding a question around 6pm. She told me that she couldn’t access info regarding the night until their system switched over for the night and I would have to wait until she finished with a patient (she was sitting at her desk at the time). We waited over an hour and she ended up leaving her shift without addressing the question. It’s so bad there that in the last transport to a dr appointment the driver stated to me that he had put in his notice due to the way everything was handled and the way people were treated. Basically we never heard from anyone in any dept unless we reached out first. We ended up asking for discharge and decided that home health would be a better option after speaking with the surgeon and learning that these issues are a common occurrence in this facility. Bad thing is, after looking at reviews it seems to be pretty standard practice there and they don’t care enough to try to correct the subpar staff and level of care. I repeat…..DO NOT SEND ANYONE HERE.!!!!!

My family member recently stayed at Brushacreek Post-Acute for rehabilitation after a hospital stay, and overall, we had a good experience. The facility was clean and well-maintained, and the staff made an effort to keep the environment calm and supportive. The nursing staff was generally attentive, especially during the daytime shifts. The therapists were excellent — they really pushed for progress but were also compassionate and patient, explaining each step of the recovery process. Communication between departments could be a little smoother at times (we occasionally had to repeat information or clarify medication schedules), but any issues we brought up were usually addressed within a day or two. The rooms were comfortable, and the food was better than I expected for a post-acute facility — my family member appreciated the variety and portion sizes. I also liked that visiting hours were flexible and that the social worker took time to help us plan the discharge and coordinate follow-up care. Overall, Brushacreek Post-Acute isn’t perfect, but it’s a solid choice for rehab or short-term recovery. The caring staff and clean environment made a difficult situation much easier for our family.
